Usefulness of Breast MRI for Evaluation of the Augmented Breast with Interstitial Injection

Abstract
Purpose: To investigate the usefulness of breast MRI in the evaluation of interstitial injection granulomas and ability to differentiate the cancer from the granulomas in the augmented breast with interstitial injection.

Materials and Methods: From Jan 1997 through Feb 2006, sixty-seven breast MRI were performed in 61 women with history of interstitial injection. Fifty-six patients previously performed mammographies or breast ultrasonographies before MRI, but breast parenchyma could not be clearly visualized. Breast MRI protocol included axial T1, T2 weighted images and dynamic contrast-enhanced T1 weighted images.

Results: The interstitial injection granulomas showed low signal intensity on T1 weighted images and variable signal intensities on T2 weighted images. Thirteen patients had abnormal lesions in background of interstitial injection granulomas. Ten ones showed centrifugally enhancing lesions or no enhancing lesions, and four of these lesions revealed no interval changes on follow-up images or benign cytologies. The malignant lesions were found in three patients and demonstrated strong early enhancement and delayed wash-out on dynamic study. Two of them were confirmed to invasive ductal carcinomas and the other one was lost during follow-up.

Conclusion: Breast MRI is superior to ultrasonography and mammography in evaluation of breast parenchymal detail in background interstitial injection granulomas. With post-contrast dynamic sequences, breast MRI is successful in identifying the tumor from in background of interstitial injection granulomas.
